Emergency Contact Card

Hi All,

Martin mentioned the importance of carrying an emergency contact card in another thread:

The link that Martin sent leads to a card to be printed and filled out by hand. That is all fine and well for those of us who have legible hand writing. I have a hard time reading my own writing so I made an Acrobat form that I filled out on my computer:

I added a field for the BC health/service number.

You may want to edit some of fields that have been pre filled....

It works for Martin and Ann who generously agreed to try it out. I think they had to save the document on their computing device before filling it out and printing it.

My card is now in my wallet. Laminating it using packing tape was the hardest part :)

Fingers Crossed that the cards are useless.

Ha ha! I thought I was the only one cheap enough to laminate with packing tape!
