I did repeats on the Molenberg this morning. The link is a 9 min clip, but you'll see all you need to see in the first 1 min. By the 5th (and last) repeat, I figured out that a tall gear is best. Despite the max. 14% grade, you want your pedal strokes out of synch with the cobbles. It's like riding on a fast wack-a-mole game. If you watch the video, you'll see that the cobbles don't last that long, but it's an intense few minutes - the front and back wheels take turns touching the road, rarely both down at the same time.

OK, those guys were a lot faster than me, so it doesn't look quite as dramatic as I described it.
